PARISH FACILITY SCHEDULING & SET-UP FORMS
Various organizations and ministries occasionally need to use rooms in our parish facility for their activities/events. Please fill out the forms below at least two weeks before the event and submit. Hard copies of the forms can be faxed to 630.543.4672 or e-mailed to bnicolai@stphiliptheapostleparish.com or mailed to St. Philip the Apostle Parish/Attn: Barb Nicolai at 1233 W. Holtz Avenue in Addison, IL 60101. The on-line scheduling form can also be submitted electronically.
